What is an individual psychosocial intervention?

Individual psychosocial intervention is personalized support designed to assist individuals in their recovery, integration and personal development. Among the many facets of psychosocial intervention are:

  • support in accessing public, private or community resources;

  • support in managing life transitions;

  • development of communication and relational skills;

  • reinforcement of autonomy in administrative and personal processes;

  • regaining power over difficulties experienced between oneself and difficult aspects of daily life at your own pace and in an anti-oppressive manner;

This intervention helps to improve quality of life by responding to concrete needs and supporting the person's independence and well-being. The work we do aims to understand your current situation and find ways to achieve well-being according to your needs and pace. Our social workers are committed to treating you with humanity, non-judgment and the utmost respect.

List of issues for which individual social work consultations are available:

  • Support or assistance in accessing a gender affirmation process;

  • Coming out and identity exploration;

  • Addictions;

  • Building self-esteem and self-confidence;

  • Relationship difficulties;

  • Managing emotions;

  • Managing issues surrounding neurodivergence;

  • Stress and anxiety management;

  • Social isolation or feelings of loneliness;

  • Mental health problems;

  • Support in accessing resources;

  • Trauma, PTSD;

  • Life transitions (marriage, separation, bereavement, change of job, job loss, moving, etc.);

  • Eating disorders; Violence and abuse. )

First appointment (up to two if necessary): assessment of social functioning

This is the process during which we will discuss the many facets of your life, your relationships, the multiple contexts and environments that shape you. The social worker will create an analysis of the interaction of all these aspects and guide you in understanding what feeds the issues you face.

Appointment(s) following assessment of social functioning: the intervention plan

Following the assessment, the social worker will present his/her understanding of your situation (his/her analysis) and his/her professional opinion (what could be done?). This is the moment when you can agree, disagree or adjust certain information if necessary, in order to create a game plan (intervention plan). The intervention plan is drawn up jointly, because we believe that you are the expert on your situation. The social worker will ask you about the goals you'd like to set for yourself, and will discuss with you how to get there.

Following appointments

Follow-up appointments will enable you to put the intervention plan into action and work towards your goals, according to your priorities and at your own pace. All this will be done in a climate of compassion, empathy and acceptance of your situation, without judgments and with openness on the part of your counsellor. The counsellor will also make sure to evaluate your objectives with you and recalibrate them as needed to motivate you to move forward towards your goals.
